Relay reached a state an agent cannot clear on its own. The response carries a code naming which one — a stuck loop, a policy decision, a missing identity — and every one of them is a judgement an agent should not make for you.

Clears when

A person reads the finding and acts. The loop resumes from wherever they leave it.
