Relay plans a task as subtasks, and each one names the files it may change. This subtask changed something else. The plan stops describing the work the moment that is allowed, so Relay refuses the subtask instead of measuring whatever came out.

Clears when

The out-of-scope changes are reverted, or the plan's scope is corrected.
